When you register you will also need to fill out a medical questionnaire, please click the link below. This is because it can take a considerable time for us to receive your medical records. Please do this as it is helpful to have up to date information about you and also to find out if you are a carer or a Military Veteran. If you need or would prefer a paper copy please pop into reception and collect one.

Moving to the UK
From the 1st April 2022, if you live outside of the below practice area it may be possible to register you or keep you registered if you move out of the area (still in Dorset). The main thing to consider when wanting to be registered with a GP practice not near to your home address is if you need a home visit. Our GPs cannot provide a home visit to a patient outside of our area. These visits will be provided by 111. When you register, or change your address to out of our area, we will send you a letter confirming that we cannot provide home visits. A copy of this is below. Our receptionist may have to ask a GP for advice if you have complex medical needs or condition.
