As well as specific practice details such as opening hours and how to register, you’ll find a wealth of useful pages covering a wide range of health issues along with links to other relevant medical organisations.
We are working to improve access to appointments for our patients, in particular at evenings & weekends. If you would like to be offered one of these appointments please ask. These appointments may be at a different site such as Poole General Hospital or Wimborne Hospital

CHANGES TO THE PRESCRIBING OF GLUTEN FREE PRODUCTS
The legislation surrounding the prescribing of gluten free products has changed so you may notice some changes to your repeat list. Bread and mixes are still available. More information and the responses to the public consultation on gluten free food regulations can be found HERE
ENHANCED DATA SHARING MODEL (EDSM)
Enables us with your consent to share your medical records with those in the NHS who are involved in your care. As EDSM has been designed to enhance patient care we will automatically 'opt you in' to both parts of the scheme. If you wish to 'opt out' then you will need to complete a consent form which you can collect from reception.

DORSET CARE RECORD (DCR)
The DCR is a confidential electronic record & currently Hospitals, Social Services & GP Practices in Dorset hold information about you on various computer systems & sometimes this information may be duplicated. The Dorset care record will join up these systems to create a more complete & up-to-date health record about you.

Every patient at our surgery will have a named GP. You can still book in to see your GP of choice. If you do not know who your GP is and you would like to, please ask at reception.

Did you know? In 2011 we offered 130,931 appointments with chiropody, eye clinics, nursing team, health visitors, blood tests, ECGs and doctors. In 2013 we offered 141,191. This year we are anticipating a further rise in the number of appointments offered at our surgeries. We always strive to meet the needs of our patients as best we can. We are pleased to offer you the facility to use “SystmOne Online” which provides internet services for patients over the age of 16 years to:
View, book or cancel doctor appointments, and
View a list of your current medication and send repeat prescription requests.
View some information from your health records
To register for this service, and be issued with a user login, please call the practice to ensure one of the ITQA team will be available to register you. You will need to bring to the practice two pieces of ID including one photographic identification, e.g. passport, driving licence and a completed registration form, which can be downloaded below.
For security and confidentiality reasons we can only give logins to the individual in person and we only offer the service for patients aged 16 and over or proxy access for parents for children under 11 years old. No access is given for patients aged 11-16 years old. Login information will not be posted, emailed or faxed to anyone and will not be given out over the telephone.
