Please be advised that The Hadleigh Practice is operating a New Telephone Triage service for urgent on the day appointments.
A Telephone Triage appointment is when a Clinician will call you by phone to discuss your medical
condition(s). They will then discuss the relevant medical advice with you, which could include collecting a prescription, having blood tests, trying some over the counter medicines etc. If the Clinician feels that a face to face appointment is required, they will then arrange this with you & ask you to come into the surgery & see a clinician later that day.
Be prepared as our receptionists will ask you why you require an urgent appointment, they are not being nosey. Our GP’s have requested that our staff ask these questions, in order to book you with the relevant clinician, that can deal with the condition you present with.
Although our receptionists are not clinically trained they are trained in the type of questions being asked.

We are working to improve access to appointments for our patients, in particular at evenings & weekends. If you would like to be offered one of these appointments please ask. These appointments may be at a different site such as Poole General Hospital or Wimborne Hospital

NEW LEG CLUB
The Hadleigh Practice along with Canford Heath Group Practice & The Birchwood Medical Centre have set up a Leg Club in the community for registered patients. The overriding objective of the Leg Club is to protect and preserve health amongst those experiencing, or at risk of experiencing, leg ulcers and/or associated conditions. We will be running a weekly session on a Tuesday 9.00 - 12.00 (last admission for treatment 11.30) at the Broadstone Youth Club, situated in Moor Road, Broadstone.

The Leg Club would be run by FOP/PPG & not by the GP Practices, the practices would provide the expertise of Nurses, HCA’s & Administrators. We would then rely on FOP/PPG members to come along to help support this Leg Club, by helping with making drinks, talking to patients & assisting where needed. We are hoping that we can have several FOP/PPG members being involved from each of the practices to help move forward with this most valuable service & also be able to continue to raise funds to run this service.
We are currently looking to recruit a Chair Person, Secretary & Treasurer along with other volunteers to become members of this group.

Enables us with your consent to share your medical records with those in the NHS who are involved in your care. As EDSM has been designed to enhance patient care we will automatically 'opt you in' to both parts of the scheme. If you wish to 'opt out' then you will need to complete a consent form which you can collect from reception.

DORSET CARE RECORD (DCR)
The DCR is a confidential electronic record & currently Hospitals, Social Services & GP Practices in Dorset hold information about you on various computer systems & sometimes this information may be duplicated. The Dorset care record will join up these systems to create a more complete & up-to-date health record about you.

We are pleased to offer you the facility to use “SystmOne Online” which provides internet services for patients over the age of 16 years to:
View, book or cancel doctor appointments, and
View a list of your current medication and send repeat prescription requests.
View some information from your health records
To register for this service, and be issued with a user login, please call the practice to ensure one of the ITQA team will be available to register you. You will need to bring to the practice two pieces of ID including one photographic identification, e.g. passport, driving licence and a completed registration form, which can be downloaded below.
For security and confidentiality reasons we can only give logins to the individual in person and we only offer the service for patients aged 16 and over or proxy access for parents for children under 11 years old. No access is given for patients aged 11-16 years old. Login information will not be posted, emailed or faxed to anyone and will not be given out over the telephone.
