CHANGES FROM 1st JULY 2017
Dr Chris Humphrey - Will become a full time Partner, working at Hadleigh House, Broadstone
Dr Rebecca Brickwood - Who has been carrying out a Maternity Locum, will be staying with us as a Salaried GP doing 6 sessions per week mainly at Hadleigh Lodge, Corfe Mullen
Dr Andrew Purbrick - Has accepted the opportunity to take up the post of Medical Director with Wessex LMCs, a statutory body which represents Gp's at local & national level. As such he will be available on a Monday & Friday
Dr Joanne Hadley - Will be reducing her sessions & will be available on Monday, Wednesday & Friday
ENHANCED DATA SHARING MODEL (EDSM)
Enables us with your consent to share your medical records with those in the NHS who are involved in your care. As EDSM has been designed to enhance patient care we will automatically 'opt you in' to both parts of the scheme. If you wish to 'opt out' then you will need to complete a consent form which you can collect from reception.

We would like to remind patients that it is your responsiblity to ensure we have your up to date contact details, including current address and telephone numbers.
Please contact reception to update your details.
Every patient at our surgery will have a named GP. You can still book in to see your GP of choice. If you do not know who your GP is and you would like to, please ask at reception.

Did you know? In 2011 we offered 130,931 appointments with chiropody, eye clinics, nursing team, health visitors, blood tests, ECGs and doctors. In 2013 we offered 141,191. This year we are anticipating a further rise in the number of appointments offered at our surgeries. We always strive to meet the needs of our patients as best we can. We are pleased to offer you the facility to use “SystmOne Online” which provides internet services for patients over the age of 16 years to:
View, book or cancel doctor appointments, and
View a list of your current medication and send repeat prescription requests.
View some information from your health records
To register for this service, and be issued with a user login, please make an appointment at one of our online services registration clinics. You will need to bring to the appointment two pieces of ID including one photographic identification, e.g. passport, driving licence and a completed registration form, which can be downloaded below.
For security and confidentiality reasons we can only give logins to the individual in person and we only offer the service for patients aged 16 and over or proxy access for parents for children under 11 years old. No access is given for patients aged 11-16 years old. Login information will not be posted, emailed or faxed to anyone and will not be given out over the telephone.
