District Nurses
The district nurse team are now based off site. To contact them please call their liaison team on 01202 705600. The team incorporates a mixed-skill nursing staff who offer nursing care to patients who are completely housebound and unable to attend either one of the surgeries.
The team are able to support patients in their own homes with the following services:
- Assessment of needs
- Wound management
- Continence care
- Palliative care
- Chemotherapy/radiotherapy support
- Venepuncture (blood testing)
- Drug therapy management
- Pain management
- Post surgical/hospital discharge care
- Signposting to other agencies
- Chronic disease support and management
- Health education and health promotion
- Equipment requisition to help maintain independence
